Hospital admission process

Admission counter is on the first floor
Please bring the following items to the admission counter on the first floor.
1. Admission form (please fill in the required items for all documents)
2. Health insurance card
3. Patient ID card (if you already have the ID card)
4. Claimant certificate for the elderly or publically funded healthcare benefits (if you have one)
5. Eligibility certificate for high-cost medical care benefits (if you have national health insurance)
6. An application for a special in-patient room (when you use a special in-patient room)
7. Discharge letter (if you were in another hospital within the last three months)

Information of Medical Fee

At Tokyo Bay Urayasu Ichikawa Medical Center, all care for patients who have neither nationality of Japan nor Japanese health insurance qualification is charged at a rate of 20 yen per one medical remuneration point.
